CREDIT REPORT AUTHORIZATION

Authorization is hereby granted to Antler Home Mortgage LLC, to obtain a consumer credit report through a credit reporting agency chosen by Antler Home Mortgage LLC. I understand and agree that Antler Home Mortgage LLC intends to use the consumer credit report for the purposes of evaluating my financial readiness to obtain a loan.

I understand that this credit report will be retained on file at the Antler Home Mortgage LLC office for use only by Antler Home Mortgage LLC staff. This information will not be disclosed to anyone by Antler Home Mortgage LLC without my written consent.

Furthermore, I understand that, should I choose to apply for financing through Antler Home Mortgage LLC, a revised credit report costing an additional fee may be required.

My signature below authorizes the release to the credit reporting agency of financial information which I have supplied to Antler Home Mortgage LLC in connection with such an evaluation. Authorization is further granted to the credit reporting agency to use photostatic reproduction of this form if required to obtain any information necessary to complete my consumer credit report.

CHECKING THE BOX BELOW GRANTS PERMISSION FOR THE RELEASE OF FINANCIAL INFORMATION TO THE CREDIT REPORTING AGENCY AND GRANTS PERMISSION FOR Antler Home Mortgage LLC TO OBTAIN A COPY OF YOUR CREDIT REPORT.

Hardware and Software Requirements: To access and retain the Disclosures electronically, you will need to use computer software and hardware that enable you to access the Internet, e-mail and the worldwide web. You must have an e-mail address that can receive mail from Antler Home Mortgage LLC.

Consenting To Do Business Electronically: Before you decide to do business electronically Antler Home Mortgage LLC you should consider whether you have the necessary computer access.

Withdrawing Consent: You may revoke or modify your consent by notifying Antler Home Mortgage, LLC at 9490 Cottontail Drive Minnetrista, MN 55375. Such change, revocation or modification must actually be received and acknowledged in order for it to be effective. If you decide to withdraw your consent, the legal validity and enforceability of prior electronic Disclosures will not be affected.

Changes To Your Contact Information: You should keep us informed of any change in your electronic or postal mailing address. You may contact Antler Home Mortgage LLC at 9490 Cottontail Drive Minnetrista, MN 55375 regarding any such change.
